Check duplication before publication, not only at prompt generation. Compare the intended reader problem, title and substantive answer against the existing corpus. A new wording does not necessarily create a new useful page.
Keep rejected candidates and their reasons so the next run can choose a different gap. A publishing quota should not override the quality gate. If the source queue is exhausted, improve an existing discussion or wait for a real question instead of disguising a duplicate as another topic.
